The carotid space (also called the carotid sheath space) is a paired deep cervical space that extends from the skull base to the mediastinum, enclosing the major neurovascular bundle of the neck. It is clinically and radiologically important due to its role as a conduit for infections, tumors, and vascular pathologies.

Boundaries

• Anterior: Buccopharyngeal fascia (posterior to pharynx).
• Posterior: Prevertebral fascia.
• Lateral: Superficial layer of deep cervical fascia.
• Medial: Retropharyngeal space and visceral space.

• Superior: Jugular foramen at skull base.
• Inferior: Thoracic inlet → continuous with mediastinum.

• Vascular: Internal carotid artery (ICA), common carotid artery (CCA), internal jugular vein (IJV).
• Neural: Cranial nerves IX, X, XI, XII (as they exit skull base), sympathetic chain.
• Lymphatics: Deep cervical lymph nodes.
• Other: Carotid body.
