Paget disease of the breast is a rare form of intraductal adenocarcinoma that involves the epidermis of the nipple–areolar complex, usually associated with an underlying Ductal carcinoma in situ (DCIS) or Invasive ductal carcinoma (IDC).

It accounts for 1–3% of all breast cancers.

Mammography Negative mammogram does not exclude disease
• Subareolar or retroareolar mass
Microcalcifications (linear, branching if DCIS)
Skin thickening or nipple retraction
US Subareolar mass
Ductal ectasia
Axillary lymphadenopathy
• Hypoechoic or heterogeneous mass under the nipple
MR Indicated especially when mammogram/US are normal. High sensitivity for multifocal/multicentric disease.
Enhancing retroareolar mass or non-mass enhancement
• Skin thickening or nipple enhancement
• Segmental/linear enhancement pattern (DCIS)
