The Ranke complex is the healed and calcified form of the Ghon complex, consisting of:
It represents past primary pulmonary tuberculosis (TB) that has undergone fibrosis and calcification, usually in an immunocompetent host.
| Modality | Imaging features |
|---|---|
| CXR | • Calcified peripheral (subpleural) nodule – often in mid or lower zones. |
| • Calcified ipsilateral hilar or mediastinal lymph nodes. | |
| • No active parenchymal infiltrates. | |
| • No cavitation (helps differentiate from reactivation TB). | |
| CT | • Dense calcified parenchymal nodule near pleura. |
| • Calcified lymph nodes in the hilum or mediastinum. | |
| • Surrounding lung may show subtle fibrosis or volume loss. |

| Condition | Differentiating Features |
|---|---|
| Healed histoplasmosis | Often multiple smaller calcified nodules; endemic areas |
| Silicosis | Eggshell calcification of lymph nodes; occupational history |
| Sarcoidosis | Bilateral hilar adenopathy, non-caseating granulomas |
| Old varicella pneumonia | Multiple diffuse punctate calcifications |
