A unicornuate uterus is a rare Müllerian duct anomaly that arises from incomplete or failed development of one of the paired Müllerian ducts, resulting in a single, banana-shaped uterus.

It accounts for approximately 5% of Müllerian anomalies and is often associated with a rudimentary horn, which may or may not communicate with the main uterine cavity.

B Unicornuate uterus with non-communicating horn
(may have functional endometrium → risk of hematometra, endometriosis)
C Unicornuate uterus with no horn
D Rudimentary horn without cavity (fibrous)
